As the RemStar Auto with C-Flex has automatic adjustment, only the section of the prescription form pressure settings that says "If RemStar Auto" needs to be filled out. All of the other pressure settings are taken care of by the RemStar Auto itself.

Alternatively you can print a copy of the form, have it filled in and then fax it to 0844 504 9494. You may also scan any medical paper or sleep study results you have and send those to us. You may place your order for a CPAP machine online and then send your paperwork. They will be matched by our staff. If you do not know your CPAP pressure (ie you do not know exactly what your pressure setting is) an AUTOMATIC CPAP will be recommended for you.
